HCBA Waiver

Waiver agencies are currently placing all HCBA applications onto a waiting list. Applicants are being processed for enrollment at a limited capacity. DHCS provided guidance to all waiver agencies on which applications will be prioritized for an enrollment slot; under 21 applicants and those in a facility for at least 60 days. The timeframe for adults in the community to enroll is taking over a year or longer. Please allow our staff to contact you with additional information.

Home and Community-Based Services (HCBS) Waivers allow for creative alternatives for those who would otherwise require care in a nursing facility or hospital. Medi-Cal, in an agreement with the U.S. federal government, allows for waiver services to be provided in either a home or community setting. Provided you have full-scope Medi-Cal eligibility, you can receive services under the waiver, which must cost no more than the alternative institutional level of care.

Services can include:

  • Case management
  • Community transition services
  • Private duty nursing
  • Home health aides
  • Family training
  • Life-sustaining utility reimbursement
  • Respite care
  • Habilitation services
  • Any other services necessary to maintain the health and safety of participants in the setting of their choice

Put another way, the HCBA Waiver provides care management services to those who are at risk for nursing home or institutional placement. Provided by a multidisciplinary care team consisting of a nurse and social worker, the care management team coordinates the HCBA Waiver as well as State Plan services, such as medical, behavioral health, and in-home support. It also arranges for other long-term services and support systems that are available in the local community, for as long as they are medically necessary.

You can receive care management and HCBA Waiver services in any community-based residence, whether that’s your own privately owned home, the home of a family member or one that is secured through a tenant lease arrangement.

The Department of Health Care Services (DHCS) Integrated Systems of Care Division recently announced Libertana as the designated Waiver Agency for the San Mateo county to assist with the exit of Institute of Aging in that area.

Additionally, DHCS designated Libertana as the Waiver Agency for the previous DHCS managed counties of Alpine, Inyo and Mono.

These territories add to Libertana’s existing service areas including;

Amador, Calaveras, Fresno, Kern, Kings, Madera, Mariposa, Merced, Monterey, San Benito, San Luis Obispo, Santa Clara, Santa Cruz, Stanislaus, Tulare, Tuolumne and sections of Los Angeles and Orange Counties​
